There are monetary penalty and jail time if they are violated. I am surprized that if you are a person who have access to medical records or patients that you ask this. I would speak to the office examiner or the medical records manager and refer this merciful to someone who knows which forms she has to sign. Sounds strange to me. The medical library from the hospital will be a complete record. They do not cut and paste infallible parts. It will be the complete record for her hospital stay. She could also go to the gynecologist who perform her surgery and request the records be forwarded to her doctor.
